Becoming A Family Day Care Provider
The first step that one should take in order to become a provider of a Family Day Care Center is to obtain a license from the NYC Department of Health Service at:

Wait for a response from the program to receive kids in you home
If you are a parent that wants your child to enter the family daycare program:
Parent must work 20 to 40 hours a week
Financial eligibility is determined by a family's gross income, with consideration of family size. Fees for child care services are based on a sliding scale net income can not exceed $33.000
The child must have a age between 2 months and 12 years of age
The child must be referred by ACS to our program
Yearly physical must be completed when the child enter the program, and must be renewed yearly for as long as the child stay at the program.
