Job Description:

Acts as patient liaison between patient and professional staff

Surveys the community outreach program and recommends changes modifications.

Ascertains patient socio-environmental needs and help themselves through a coordinated system of referrals and advocacy

Member of the Quality Assurance Committee

Participates in intakes and evaluations as a member of the clinical team

Engages on research and follow-ups activities to evaluate counseling techniques

Provides individuals counseling and supportive services to the patient and relatives

Develop and implant Patient treatment plan

Maintains patient records as per policy and procedures

Prepares reports as per established policy

To participate actively in all in-service training sessions and workshops for staff

Participates in community educational programs to develop favorable attitudes towards recognition and treatment with emphasis on prevention and general education

To assist all staffing establishing safe and healthy environment

Conduct educational group and supervision

Conduct to clinical treatment team

Write reports on activities and prepare for future activities

Maintain patient case record in compliance

Carries out other duties assigned
